Placement Support Officer – Sourcing Care

Somerset County Council

A Bit About Us

Our Adult Social Care Service has an exciting 18 fixed term opportunity for a motivated Placement Support Officer within its Sourcing Care function. This post is available due to promotion within the service. Our sourcing care team is expanding and it’s a brilliant time to join us. The team deals with all enquiries and service requests relating to bedded placements, including residential, nursing, and complex care. We play a vital role in ensuring those we support are able to secure the care they require in a timely and efficient way.

If you have experience in a residential nursing home, hospital discharge or adult social care setting or as a Senior Carer in a care home, and are looking for the next step in your career, we would love to hear from you.

We recognise the importance of a good work life balance and do everything we can to accommodate flexible working, including some working from home, compressed hours, and other arrangements. Please just let us know in your application or at any stage throughout the process (and beyond) if these are options you’d like to explore.

Here’s what you can expect to be doing

Working within our supportive, busy team, as a Placement Support Officer you will liaise with staff and managers across a range of agencies and forums (including statutory health and care services, our independent care provider market, and the wider voluntary and community sector) to actively coordinate high quality, person-centred care and support for individuals in an efficient manner.

A Few Things About You

To join us as a Placement Support Officer you will:

  • Have some knowledge of the statutory, regulatory and policy framework within which care sourcing/brokerage functions operate and a sound understanding of adult social care.
  • Be able to communicate and negotiate with confidence, problem solve, motivate and effectively influence others, to obtain the desired service outcome for individuals we support.
  • Have some prior supervisory experience.
