About The Role

We’re currently looking for a Homeless Services Care Manager for our integrated primary care office, the Pittsburgh Mercy Family Health Center to serve on our care management team. Our care managers have in-depth knowledge and experience in the care coordination of individuals experiencing serious and persistent mental illness, substance use disorder and/or housing instability.

The Homeless Services Care provides brief focused interventions, motivational interviewing and crisis intervention to health center patients while working in coordination with the patients treatment team or while working to get the individual in to a higher level of care if needed. Our care managers develop trusting relationships with persons served through high-touch engagement in the primary care office and utilize patient registries to track our high risk patients.

Requirements

  • Master’s Degree in Human Services and 1 year of community-based health services deliver or a Bachelor’s Degree in Human Services and 3 years of community-based behavioral health service delivery
  • Work Schedule: 37.5 hours per week, typically Mon to Fri 8am – 4pm (this is an hourly, non-exempt position)
