Title: Health and Social Care Caseworker Area Coordinator

Location: Beverley and surrounding area

Salary: £25,357 per annum pro-rata, based on 35 hours per week

Contract Type: Fixed Term Contract until 30th September 2023 with a possibility of an extension to January 2024

Could you ensure our Health and Social Care service is delivered effectively, consistently, and exceeds quality standards?
You will be working on the innovative Area Coordination (AC) programme, with individuals who currently, or may need to, access emergency or formal care. System pressures require a different way of working, with a focus on prevention and community development, so that sustainable outcomes can be achieved.

Working across projects in six defined areas, you will deliver person centred, strength-based interventions that improve self-management, build independence and resilience, whilst preventing a need for formal care .

You will be working intensively with a rolling caseload, helping people to identify and address unmet social needs that may be exacerbating physical and/or mental health. The people we support may have complex clinical presentations, (including mental health, substance misuse, multiple long-term conditions, trauma) and they may be isolated and suspicious of formality and offers of support. So, we are looking for someone whose ability to connect is exceptionally strong. You must be authentic, relatable and compassionate, and able to think on your feet.
You will also need to build strong relationships with many partners, including adult social care teams, primary care, social prescribers, housing and the local voluntary sector. Together you will advocate for equity of access and improve integrated working.

Lastly, as most of your time will be spent out in the community; meeting people in their homes, in cafes or walking in the park, so you will also need to be confident of lone working procedures and able to keep meticulous support records whilst on the hop.

Previous experience of asset-based community development (ABCD) would be highly advantageous to your application.

Due to the specific requirements of this role, there is a need to visit various locations, some of which are remote with no access to public transport, therefore you must have access to a vehicle which you are willing and able to use in conjunction with your duties.
