You are a Shared Lives Officer experienced in working in the community or social care with people with disabilities, possessing excellent communication and interpersonal skills; a friendly approach; and the ability to work in a number of different environments.

You will bring your Diploma Level 3 in Health & Social Care (or willingness to work towards one if you hold a Level 2 in Health & Social Care) to a council where we put Swindon and its people at the heart of everything we do.

Your next role

A varied role in a well-established team, working 30 hours per week across 4 days, with responsibilities such as carrying out continuous assessment of needs, aims, objectives and outcomes while accurately complete forms and maintain records.

Here you’ll case manage providers of accommodation and support in the community as part of the Shared Lives team responsible for supporting adults with learning disabilities, autism, physical disabilities and mental health needs and their Shared Lives Carers.

This role will see you providing personalised support with positive approaches to individuals across the service using proactive and reactive strategies to manage the support of people who challenge services while working with multidisciplinary teams in your locality to promote independence in the community.

You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act “for the effective performance of a customer-facing role”).

As part of the COVID19 strategy this role will be subject to regular covid19 testing, as per the guidance set out by the Department of Health and Social Care.

From 11 November 2021, new government legislation requires people working in a Care Quality Commission-registered care home for adults, to have had two doses of a COVID-19 vaccine to enter the premises (unless they have a medical exemption).
