Salary: £18,972 based on 35 hours per week (Monday to Friday 9am – 5pm)
Contract Type: Fixed term until March 2024

Have you ever considered what happens to patients when they’re discharged from hospital? Some people are ok and have what they need. Others may benefit from a little more time and support. That support may include collecting prescriptions, making sure they have food in the house, light household tasks and making sure they know who to contact should they need more help – The general support you would want your own friends and family to experience.

That’s what this job is all about – helping patients who have been discharged manage and reduce the risk of needing another hospital visit.

Your days will be spent speaking to and supporting patients and medical staff so that the hospital can run smoothly and see patients as quickly as possible.

Your experience will allow people who have been in hospital to have their independence. The people you support will come from all kinds of backgrounds. You may spend your morning supporting someone with Dementia getting settled back at home after a hospital stay, then in afternoon you could be taking someone with autism to a doctor’s appointment for a follow up. You might end your day dropping some groceries in to somebody who can’t leave the house after an operation. This role will have a huge impact on our service users’ day.

It’s important to know – This role does not involve personal care. You will not be responsible for bathing or cleaning our clients.

A day in the life of a Support Worker will involve:

Speaking with healthcare professionals about the support patients require
Driving to different locations including hospital, doctor surgeries, pharmacies, shops and patient’s homes.
Offering both emotional and practical support to people using our service.
Ensuring our systems are updated so that we can track what support has been offered to patients.
Helping to assess our service users future needs and helping develop support plans.

To be a successful Support Worker, You’ll need:

A driving licence and access to your own vehicle.
A caring, empathetic nature to be able to offer the best support.
Good IT skills as we use software to ensure our services are streamlined and efficient.
An understanding of the services the NHS provide, to be able to offer the best advice and guidance to our service users.

The closing date for applications is 11.59pm on Tuesday 4th July 2023 with interviews expected to take place on Monday 10th July.
