TERMS OF REFERENCE for individual Consultant

Programme Advisor for MdM Germany mission in Ukraine

I. INTRODUCTION

The humanitarian aid organization Ärzte der Welt e. V. is the German division of the international network Médecins du Monde/Doctors of the World (MdM). The 17 members of the network implement domestic and international health care projects, currently almost 417 projects in 74 countries around the world.

MdM provides emergency medical aid in disaster and crisis situations, supports access to health care and, if possible, assists with reconstruction measures. The network is also active in risk reduction and preparedness as well as long-term development co-operations in neglected regions around the world that receive little public attention. MdM ensures that its health services are provided equally to the most vulnerable citizens, refugees, internally displaced people and host communities, regardless of their ethnic, social, religious or political background.

MdM documents and publishes eye-witness reports, human rights violations and leads campaigns to raise awareness of important topics such as poverty, migration or the costs of specific drugs. It tries to hold decision makers accountable and to convince them to support positive changes through a variety of advocacy activities.

MdM considers access to general health care to be a universal human right for everyone.

In Eastern Ukraine MdM, targets the most vulnerable communities and people affected by the ongoing conflict by improving access and availability of health care services for the population in Donetsk Oblast, Kyiv Oblast, Chernivtsi Oblast and Dnipropetrovsk Oblasts namely internally displaced persons (IDPs) and host communities in government-controlled areas (GCAs). Currently three MdM chapters (MdM Germany, MdM Spain and MdM Greece) are implementing the MdM intervention in Ukraine.

II. CONTEXT

Due to the Russian invasion starting on 24 February the UN estimates that 1.5 million Ukrainians fled the country within the first 10 days of the war. By the end of June, the number of Ukrainian refugees residing in neighbouring countries has risen to 5.7 million, while 6.3 million people have become displaced within Ukraine.

An estimated 15,7 million people in Ukraine are now dependent on humanitarian aid. It is feared that these numbers will continue to rise. Health care is deteriorating rapidly, and the sanctity of medical hospitals is not being respected by parties in the conflict. Civilian casualties are rising daily, as is general human suffering. On 03 July 2022, the Office of the High Commissioner for Human Rights (OHCHR) reported at least 11,000 civilian casualties, including 4,889 killed, although the actual numbers are likely much higher.

MdM has been operating in Ukraine since 2015 by implementing a comprehensive health care programme, focusing on improving access and quality of primary health care services for the conflict affected population living along the former line of Contact (LoC) in Luhansk and Donetsk Oblast.

Since 2020 the programme had been designed to serve the Humanitarian-Development Nexus. While on one hand multidisciplinary medical mobile units provided direct services (including primary healthcare services (PHC), sexual and reproductive health (SRH), and Mental Health and Psychosocial support services (MHPSS)) to most isolated and affected communities, the local health system and communities were strengthened through capacity building, awareness raising and advocacy in order to integrate outreach services gradually into the public health system.

The measures were implemented by two project offices in Severodonetsk (Luhansk Oblast, 41 employees) and Bakhmut (Donetsk Oblast, 22 employees) as well as a coordination office in Kiev (10 employees). Following the February 2022 military hostilities across Ukraine, MdM relocated its offices from Kiev and Severodonetsk to Chernivtsi and from Bakhmut to Dnipro and adjusted its projects to the current conditions:

  1. comprehensive medical care (PHC, SRH, MHPSS) for IDPs/hosting communities in selected shelters/transit centres/communities through medical mobile units.
  2. support to selected health facilities with medicines and medical supplies

III. OBJECTIVE OF THE CONSULTANCY

The consultancy aims to:

  • Help MdM Germany team to define a relevant and effective response strategy which aligns with the general strategy of MdM in Ukraine. This response strategy should include an overall objective and define programmatic pillars to support this objective.
  • Accompany MdM Germany team in conceptualizing intervention packages for the different regions (since the escalation the scope of health needs has changed dramatically and the country has been split into 3 different regions: (1) those receiving IDPs, (2) areas re-taken by the Ukrainian government and (3) active conflict zones. )that align with the overall objective of the mission, correspond to the programmatic pillars and existing e.g. BMZ) and planned donor grants.
  • Propose a mission Logical Framework, which specifies the project objectives, results, activities and SMART indicators for each programmatic pillar.
  • Develop the capacity of MdM Germany team on key concepts of Project Cycle Management (based on needs).

IV. METHODOLOGY

  1. Preliminary work
  2. Literature/secondary data review: literature review and analysis of existing data from relevant stakeholders (MdM and others)
  3. Briefing with MdM HQ
  4. Fieldwork
  5. Sites visits
  6. Workshops/ working sessions with MdM Germany team
  7. Visits and/ or virtual meetings with MdM Spain team
  8. Reporting
  9. Development of a Response Strategy, including intervention packages and mission log frame
  10. Drafting the consultancy report
  11. Debriefing with MdM HQ

The Consultant will be responsible for developing and presenting a sound methodology. The methodology will be submitted as part of the application process and will be discussed later on with MdM Germany if the application is successful.

V. DELIVERABLES

  • A concept note describing the finally agreed methodological approach as well as workplan and timetable (to be provided before the field work in Ukraine starts)
  • A strategy document detailing MdM Germany programme in Ukraine, including a mission log frame
  • Revised log frame for the project “Strengthening community and health system resilience” funded by BMZ
  • A mission report, including recommendation on team capacity development and proposition of follow up.

All documentation will be property of MdM and will be disseminated as appropriate.

VI. LOCATION OF THE ASSESSMENT

The assessment will be carried out in Dnipro where MdM Germany team is currently based, with visits to Vinnitsa where MdM Germany is planning to launch a new project.

VII. TIMEFRAME

The assessment is expected to take place in August/September 2022. The following preliminary timeframe is foreseen while a final timetable is expected from the consultant

Preparation and desk study, including briefing with MdM:3 days

Travel (from / to the Romanian border): 4 days

Work in the field, including programme and proposal writing: 20 days

Debriefing with MdM and Report writing: 5 days

TOTAL: 32 days

MdM will organise transport and accommodation to and within Ukraine and will provide administrative support.

  • 1 week after Submission of draft reports in English MdM to comment departure from Ukraine
  • 2 weeks after Submission of final reports in English to MdM departure from Ukraine

VIII. REQUIRED EXPERTISE

  • Advanced university degree in health-related field, humanitarian aid, international development or other related field
  • At least 5 years of proven experience in programme or project design, management and monitoring
  • Experience managing multi-sectoral and/or health interventions.
  • Experience in programme design for humanitarian as well as development settings
  • Deep proposal devlopment and writing experience with a variety of donors
  • Experience in team capacity development
  • Regional experience is seen as an asset
  • Fluency in English (spoken/written), Ukrainian/Russian is an asset
  • Exceptional interpersonal communication skills and demonstrable team leadership experience
