Salary: £28,821 per annum based on 35 hours per week plus Inner London weighting of £3,344, if applicable

Could you help people who have complex medical / social needs?

In line with NHS RightCare’s HIU programme, you would work across the local health and social care system.

You would be using a de-medicalised, de-criminalised and humanised approach to help prevent people from reaching crisis point. This help is also aimed at reducing admissions to hospitals and the need for residential and nursing care.

This is a person-centred service to enable people who need us to achieve their personal outcomes. This could be an enhanced quality of life, increased confidence and independence, and an improved sense of choice and control.

We are looking for an Outreach Specialist to join our team of experts in London and be a key part of our national network. Help us collaborate on best practices and spread the power of kindness!

A day in the life of an Outreach Specialist will include:

  • Working with a rolling cohort of patients who attend A&E, call 999 or have been admitted to hospital more than might be expected.
  • Seeking to appropriately reduce the number of unscheduled emergency contacts along with our HIU Community Connectors.
  • Working closely with external organisations like Accident & Emergency departments, Ambulance Service, Police, Local Authorities, local Community Health Teams and the local voluntary sector.

To be a successful Outreach Specialist:

  • Do you have experience in person centred support planning? Can you identify risks and anticipate issues?
  • Are you able to make a difference to people’s lives, by putting in place interventions that reintegrate people into the local community? This would be done without the need to make demands on services that may not be the most appropriate to meet their needs.
  • Are youresilient and motivated? Can you demonstrate compassion, strong negotiation skills, patience and a real appetite for helping some of the most vulnerable people in our society?
