Hours: 35 hours per week
Salary: £28,632 to £30,000 per annum (depending on experience) + £300 flexi working allowance

A day in the life of an Outreach Caseworker will involve:

Outreach working across Sandwell and West Birmingham to work on an innovative NHS programme that helps support people who access health care more than expected (High Intensity Use (HIU) Programme) in line with NHS RightCare’s HIU programme -using a de-medicalised, de-criminalised and humanised approach.

Connecting with people and strong enough to support a change in behaviour for even those with the most complex medical/social needs.
Working with a rolling cohort of patients who attend A&E, call 999 or have been admitted to hospital more than might be expected. You will seek to appropriately reduce the number of unscheduled emergency contacts along with our HIU Community Connectors. You will be working closely with the CCG, the A&E department, Ambulance Service, Police, Local Authorities, local Community Health Teams and the local voluntary sector.

To be a successful Outreach Caseworker, you’ll need:

• Exceptionally strong ability to connect with other people – you’ll be authentic, relatable and compassionate.
• You understand how the system works and how to help people falling through the gaps.
• You’ve experience in person centred support planning and dynamic risk assessment.
• You’re strong, resilient and courageous, with a real appetite to help some of the most vulnerable people in our society.
• Can think on your feet, with strong negotiat ion skills
• Meticulous record keeping skills
