Health Access Program Coordinator

SCOPE OF WORK

Under the direction of the Health Program Manager, the Health Access Program Coordinator will be responsible for facilitating health access for newly resettled refugees.

RESPONSIBILITIES

· Email weekly arrivals and health program spreadsheets to UDOH Refugee Health Program

· Complete monthly billings to UDOH Refugee Health Program

· Quarterly data collection and reporting to UDOH & other funders on program outcomes

· Oversee Medicaid enrollment and HMO completion

· Coordinate health intakes within 72 hours of arrival

· Ensure newly arrived refugees receive health screening within 30 days of arrival

· Schedule and coordinate health screening follow up appointments and referrals

· Coordinate the completion of all LTBI related appointments including chest x-rays, chest clinic, nurse intake, med pick up, and other LTBI related duties

· Coordinate on-going adult dental care with South Main Health Center and other partners

· Manage the weekly transportation list

· Supervise Health Access Program Specialists and Assistants

· Supervise 1-3 Health Access Interns per semester

· Assist with interpreter coordination and support

· Comply with all policies and protocols of the agency

· Support overall functioning of Health Programs, projects and initiatives

· Participate in all program meetings and staff development activities

· Complete R&P health related case notes

· Support health team staff in teaching clients how to navigate the U.S. healthcare system

· Assist clients in ensuring medical bills have been billed to Medicaid

· Other related duties as assigned.

REQUIREMENTS

· Bachelor’s degree in Public Health, Health Promotion or Social Work preferred

· Minimum 1-2 years of work experience in health education/administration; preferably with diverse/refugee population

· Familiar with Medicaid, US health care system, and refugee health needs/issues

· Excellent verbal and written communication skills in English and demonstrated success working and communicating effectively in a multi-cultural environment and with limited-English speakers

· Demonstrated ability to work independently and as a team member

· Strong computer skill with proficiency in Microsoft Office (Word, Excel, and Outlook); database experience a plus

· Well-organized with proven ability to prioritize duties and manage time effectively

· Valid driver’s license, current insurance and access to reliable transportation required
