Project Officer Primary Care Value Based Health Care

A unique opportunity for a project officer to become part of the primary care team in CTM UHB and develop a Value Based Healthcare programme to improve the health outcomes for our population and reduce the risk of stroke.

The programme will put in place a value-based approach to the prevention of ill health and promotion of good health, on a population wide basis in primary care, in relation to stroke and Cardiovascular Disease.

You will be an integral part of the team with the full support of a cross health board produced programme and the primary care team.

The Project Officer will be instrumental in the development of plans, documentation and guidance for relevant project teams together with ensuring that communication is maintained between all stakeholders throughout the project lifecycle, organisational procedures are adhered to, objectives are identified and met, and benefits realised from the implementation of the change

Some of the key responsibilities of the post will be:

Provide management support to teams ensuring projects are delivered in line with the planned and agreed timescales, delivering the functionality and benefits identified.
Persuade, influence and align others towards a common purpose to empower and inspire people to achieve project success, enabling the project to proceed in an environment of change. This will involve making complex formal presentations to large groups to achieve project delivery.
Serve as key liaison between the Programme Team and other stakeholders across Cwm Taf Morgannwg UHB.
Able to interpret large amount of complex information, and make definitive decisions.
As part of the, CTM Health Check and Optimal Management of Atrial Fibrillation (AF) and Hypertension in Primary Care Programme, identify and develop reporting tools to allow for propriety planning at Work Stream Groups.
Maintain risk, quality and issue logs, reporting on high priority issues using intelligence and presenting it in an appropriate format which includes the formal communication of project information to stakeholders.

 

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
