Detailed Job Description And Main Responsibilities

An exciting opportunity has arisen for a highly motivated and innovative Registered General Nurse to lead a Community Nursing team under the guidance of a Modern Matron & Senior Lead Community Nurse.

You will lead a team to provide the highest standard of community nursing care, putting patients first and getting it right first time.

You will provide a dedicated nursing service to include assessment, planning and delivery of nursing care in the community. This includes assessment of health needs, leading a team of staff and meeting the ever developing role of the community nurse. The successful candidate will provide support in order to prevent patients having unnecessary hospital admission, enabling them to stay at home where possible.

You will be responsible for the supervision, deployment and teaching of junior staff. You will be expected to work within the policies of the Trust and the NMC Code.

You will use a combination of skills and knowledge to identify health and nursing needs, and develop strategies to promote healthcare and independence whilst recognising cultural beliefs and values of the individual. You will be expected to liaise closely and work collaboratively with the Primary Care Network to support & treat patients within their own homes.

The successful applicant will be a 1st level nurse with community nursing experience who can demonstrate leadership and the ability to work within a changing environment, who is flexible and keen to develop as the service needs.
