**Community Outreach Manager – Health & Social Care Location: East Riding of Yorkshire (hybrid role working between home and / or our Hull office with regular travel throughout the region) Contract type: Permanent Hours per week: 35 (Monday to Friday, 9am to 5pm) Salary: Up to £40,550 per annum

About this opportunity…**

An exciting opportunity has arisen for a Community Outreach Manager – Health & Social Care to join the British Red Cross team in East Riding. You will be working on an innovative Local Co-ordination Service (LCS) programme, with individuals who access Adult Social Care; helping them to access the right services / support that allows them to better self-manage and flourish.

Working across projects in a defined area, you wil l manage specialist teams who will deliver person centred and strength-based interventions that improve self-management, build confidence, independence and resilience whilst preventing a need for formal care.

We are looking for someone with exceptional management skills and confidence, with experience of coaching a team to think outside the box to find solutions for people with varying levels of complexity (including mental health needs, drug and alcohol use and multiple long-term conditions). You will be able to demonstrate how to develop person centred support plans, manage risk dynamically and access and build assets in the community.

You will be responsible for the safe delivery of services, embedding a quality improvement approach and shared learning culture. You will also use data and service user feedback to identify gaps in services that would prevent escalation of crisis whilst finding interrelated outcomes, that would promote the value of the serv ice as a recovery and prevention pathway.

You will be required to manage relationships with system partners, commissioners, and local community providers. You will develop strategic relationships, looking for opportunities to improve joint working, develop new referral pathways, whilst also attending multi-disciplinary team meetings, to ensure that safe and effective interventions are made.

This role may be f or you if…

You are a resilient and motivated individual with strong management skills and health and social care system knowledge who is able to demonstrate compassion, strong negotiation skills, patience and a real appetite for helping some of the most vulnerable people in our society.

You have previous experience managing a large team over a dispersed area, along with casework / outreach experience delivering community-based services across any of these areas: mental health, drug and alcohol, physical health conditions, elderly, minority cohorts.

You have experience managing safeguarding issues and processes and of working with individuals who access services frequently or in asset-based community development (ABCD)

Due to the specific requirements of this role, including the need to visit various locations, some of which are remote with no access to public transport, you must have access to a vehicle which you are wi lling and able to use in conjunction with your duties
