This position is full-time (40 hours/week) Monday – Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of (8:00am – 5:00pm CST). It may be necessary, given the business need, to work occasional overtime.

We offer 2-4 weeks of paid training. The hours during training will be 8:00am to 5:00pm Monday-Friday.

Training will be conducted virtually from your home.

If you are located within a commutable distance of Sugarland, TX, you will have the flexibility to telecommute* (work from home) as you take on rewarding challenges.

Primary Responsibilities

  • Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services 
  • Keep the member out of the hospital by supporting regular visits to their primary physician. Conduct various assessments
  • Keep member actively engaged with their primary physician
  • Manage the care plan throughout the continuum of care as a single point of contact
  • Communicate with all stakeholders the required health – related information to ensure quality coordinated care and services are provided expeditiously to all members; Proactively engage the member to manage their care
  • Provide member education and support transitions of care
  • Help to keep members compliant with their care plans

Required Qualifications

  • High School Diploma / GED (or higher)
  • 2+ years of Clerical or Administrative work experience
  • Experience with Microsoft Outlook (send emails and set folders)
  • Experience with Microsoft Excel (update spreadsheets)
  • Access to high-speed internet at residence
  • Ability to travel up to10% of the time
  • Reside with a commutable distance of Suglarland, TX.
  • Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance

Preferred Qualifications

  • Bachelor’s Degree (or higher) in Social Work or equivalent
  • 2+ years of experience call center
  • Field Based experience
  • Experience working in Managed Care
  • Medicare and / or Medicaid experience
  • Working knowledge of Medical Terminology

  • Located within a commutable distance of Sugarland, TX
  • Dedicated work area established that is separated from other living areas and provides information privacy
  • Ability to keep all company sensitive documents secure (if applicable)
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
