Join a dynamic and successful Care Management Agency as NADAP’s Assistant Vice President of Health Home Care Coordination.

The Assistant Vice President of Health Homes Care Coordination is a vital role at NADAP. This position has autonomy for managing a community-based care management program. The AVP will lead a large team and have significant interaction with Lead Health Homes and other stakeholders. The AVP must be a strong and results-driven leader with the ability to think outside the box. Extensive experience leading teams of people and championing positive culture is essential in this role, as this position will have responsibility for the overall management of Health Homes operations. Significant care management experience is necessary for success in this role, as well as the ability to reduce cost and improve quality in a community setting. The AVP must have demonstrated results in achieving both clinical and business success.

In This Role You Will:

  • Manage Health Homes Care Coordination to ensure goals are achieved. Ensure contract and regulatory compliance of operations. Define the scope of work, oversee the implementation of services, advise staff on service delivery, monitor operations, and adjust as necessary. Develop and implement new program services. Create operating plans.
  • Interact with external business associates to develop networking opportunities, promote NADAP’s business, and ensure NADAP’s best interests and needs are met. Develop relationships with funding sources and negotiate contracts where appropriate.
  • Ensure programs and departments meet fiscal targets. Participate in creating budgets and forecasts. Adjust revenue and expense accordingly. Develop, monitor, and revise budgets in coordination with SVP. Prepare for and participate in budget-to-actual meetings. Oversee billing process for departments. Balance agency and program needs. Identify cost-saving measures.
  • Oversee the development of written policies and procedures for programs/departments to create manuals and other material documenting processes.
  • Oversee the functional development and maintenance of data collection systems e.g., database and dashboards. Develop and generate reports and statistics to analyze operations.

Requirements:

  • Master’s Degree required, preferably in a related field.
  • At least seven years relevant experience, with at least three years in Health Homes/Care Coordination.
  • At least four years of supervisory experience. Able to prioritize and delegate responsibilities.
