About The Role

The North West London ASD/LD Keyworking team is currently working across the boroughs of Ealing, Hounslow, Hammersmith and Fulham, Westminster, Kensington and Chelsea, Hillingdon, Brent and Harrow with plans to recruit to the role in Harrow. The role allows for agile working with the operational base in Hounslow but predominantly working within the local area of Harrow remotely.

The ASD/LD Keyworkers work with children and young people (CYP) up to the age of 25 and is funded by NHS England as of part of the NHS Long Term Plan.

The Purpose Of Keyworkers Is To

Work across the local systems with and for the family and young person who are on the
Dynamic Support Register (DSR) who are at risk of admission to a Mental Health setting.
Use knowledge and experience to provide system navigation for families with a degree of independence to facilitate providing family focussed support and challenge to system partners when necessary.
Coordinate support at pace to avoid admission.

 

Key Responsibilities

To act as the ASD/LD Keyworker for children and young people (CYP) with autism and / or a learning disability (LD) who have complex needs in Harrow.
To work with a small caseload of children, young people and their families supporting them to build resilience, navigate and challenge the local system. Facilitating person-centred care with timely support and interventions.
To develop effective partnership working, information sharing and collaboration across health, social care and education to improve outcomes for vulnerable children and young people with ASD/LD.
To work with key stakeholders including people with lived experience to identify gaps, inequalities and variation in the system shaping and modelling the new ASD/LD Keyworking function for this cohort within North West London Integrated Care System (across health, social care and education).

 

Your Skills

Ability to ensure all care plans are child/young person-centred with a sound understanding of the theory and principles of personalised care.
Ability to build a detailed understanding of local service provision and effective relationships with key stakeholders.
Work in a flexible manner to create personalised alternatives to traditional support.
Ability to facilitate the provision of emotional and behavioural support with informed advice to children and young people with autism/ learning disabilities and their families.
Excellent written and verbal communication and negotiation skills.

 

Professional Qualifications

Professional registration ideally within CAMHS remit such as: Social Worker, Nurse, teacher, therapist or significant experience working with children and young people with additional needs.
Relevant professional qualifications
Enhanced DBS check required children and adults.
