The Role
Our Community Health and Wellbeing Workers are a vital part of Adult Social Care and Health team. As Team Coordinator, you’ll ensure no one gets left behind – supporting each of our workers in their role and enabling them to prioritise the people and projects where they’re needed most.
Our team provides personalised support to residents and households in the Golborne / World’s End area – visiting them in-person to better understand their needs, and staying in regular contact with them to see how things progress. The Community Health and Wellbeing Worker Team Coordinator, will give administrative support to the team – creating reports, organising meetings, connecting people with relevant stakeholders, and gathering data in a safe and confidential manner.
Alongside our Community Health and Wellbeing Workers, you will help people make better, more informed decisions on their health and wellbeing – acting as a bridge between residents and local support services. This could include supporting someone through issues such as mental health, isolation, loneliness, vaccinations, diet, fitness, or chronic disease management.
You will report into the Community Health Programme Manager and work closely with community partners to better understand the various activities and services available to the local community. You will also work with GP’s and other healthcare professionals to ensure that patient needs are being met and properly communicated.

You will be a naturally organised and detail-oriented person, with great communication skills. We work with some of the community’s most vulnerable residents, and we deal with some incredibly sensitive topics on a regular basis, so a resilient mind and positive attitude will make a big difference.
You will have experience supporting teams of staff or volunteers, and you will be comfortable and confident with administrative duties like data entry and time management.
You will have at least five GCSEs (or equivalent) at C or above, including English and Science, and it would be helpful to have some basic knowledge of good healthcare practices and NHS services. A knowledge of the local community and the language spoken here could be advantageous too.
