SUMMARY:

(APM), a nonprofit IRS 501(c)3 Latino based health, human services and community development organization located at Philadelphia, Pennsylvania, is to assists families achieve their full potential in life, by empowering at-risk families in our community to make positive life choices that enable them to maximize their personal potential.

Through collaborative programming with health, human services, housing and other community-based organizations. Case management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s needs. It uses communication and available resources to promote high quality, cost-effective outcomes. The Case Manager will be responsible to ensure program quality and performance related to recruiting, screening, matching, monitoring, and closing the relationship with each program participants. FUNCTION: Under the supervision of the Supportive Housing Director the Case Manager is responsible for providing a full range of culturally and language appropriate case management services to APM Supportive Housing clients and others, assigned to their caseload.

DUTIES AND RESPONSIBILITIES:

  • Initiates the intake process for all the clients that enter the program.
  • Performs face to face housing and social assessment with all new applicants. Refers them when warranted.
  • Documents in client chart/HMIS and works in collaboration with the professional team working with applicants.
  • Obtains demographics and medical data and informed consent of each client accordingly with program procedure.
  • Develops supportive service care plans together with client and other members of support team (e.g., Primary Care Provider, mental health and drug and alcohol therapist) as needed, while ensuring confidentiality.
  • Coordinates office, home visits, etc., and provision of services, case management and advocacy in accordance with Program procedures.
  • Works in collaboration with the client support system as needed.
  • Participates in APM’s staff and supervisory meetings.
  • Participates in training, supervision and staff development as required.
  • Maintains complete and accurate, timely records as required in client’s chart, Credible or HMIS, as indicated by the program. ? Provides referral to all the needed services to clients and members of their household as needed.
  • Works with the case managers and other co-workers as a team to deliver case management services.
  • In coordination with program coordinator the case manager will establish collaborative agreements within APM services and with other community-base organizations to increase access to mental health, drug and alcohol, primary care, and to other medical related services, as appropriate.
  • Performs other duties, including housing counseling, as required by supervisor.
  • Compiles list of referrals resources for target populations to ensure continuity of housing and social stability. These referrals should include, but not be limited to, counseling, drug/alcohol, medical, family planning, housing, food, childcare, educational and legal services.

PERFORMANCE STANDARD:

  • Completes intake process as required by the case management procedure.
  • Completes documentation and paperwork on completed Intakes within 24 hours of client interview, and other contacts.
  • All paperwork on contact with clients and referrals made on behalf of clients completed per time frames established in the Case Manager Policy and Procedures Manual.
  • Documents the minimum contact with each client assigned to caseload as per time frames outlined in the Case Manager Policy and Procedures Manual.
  • Participates in bi-weekly (or as required) individual supervisory meeting.
  • Participates in monthly staff meetings (or as required).
  • Completes monthly statistical reports by the first of the month for the previous month worked.
  • Maintains awareness and knowledge of resources, benefits and entitlement necessary to refer clients for services and to advocated for client needs.
  • Eligible for targeted case management certifications (as required).
  • Complies with all APM, Inc., Health Promotions/La CASA Project policies and regulations.

Requirements:

MINIMUM QUALIFICATIONS:

  • Associate or bachelor’s degree in Social Work or related field.
  • 1 year of case management experience required. 2 years preferred.
  • Bilingual/Bicultural (Spanish/English).
  • Understanding/knowledgeable of psychosocial and medical issues related to HIV disease.
  • Knowledge of health and social resources.
  • Excellent oral and written communication skills.
