Description
Trinity Health Of New England is looking for an experienced Regional Director of Community Connect to join our team of mission-driven healthcare innovators. The Connect Program Director is responsible for strategy, leadership, operations, business metrics identification and managerial direction in the development and delivery of TogetherCare Connect to affiliate partners. Primary responsibility for the design, oversight, and day-to-day management of Electronic Health Record/Practice Management (EHR/PM) solutions for independent provider medical practices and hospitals. As the primary owner person for the business relationship at the Regional Health Ministries (RHM), the Connect Program Director builds and maintains strong relationships with stakeholder groups including independent medical practices, Clinically Integrated Networks (CIN), System Office Health Informatics, TogetherCare, Trinity Information Services and other relevant organizational entities.

Work Hours/Shift

You Will Be Responsible For
- Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions.
- Manages Connect readiness, activation, post go live support and optimization with the Connect Partner. Builds and maintains strong relationships with Connect Partners.
- Collaborates with Health Informatics, TogetherCare, Clinically Integrated Networks, Medical Groups, Trinity Information Services and Connect Partners in supporting the organizational mission, vision and goals.
- Collaborates with colleagues, care providers and business operations to establish standards of care, procedures, utilizing evidence, quality indicators, regulatory requirements, and cost and performance data to guide decision making.
- Facilitates health information technology solutions that bring evidence-based care standards and care delivery models to the system thus reducing clinical variation, unwarranted clinical variation and improving business and operational performance. Frames the implementation strategies to be operationally effective to achieve desired performance.
Requirements
- Education: Bachelor’s Degree in Health Informatics, Business Administration, Health Care Administration, or related discipline required or equivalent combination of education and experience. Master’s degree preferred.
- Experience: Must possess a comprehensive knowledge of Health Informatics, healthcare business operations a minimum of five (5) to seven (7) years of experience in a health informatics, health care administration or related role and experience with clinical information systems, health information technology and managing medical groups.
- Ability to work independently and autonomously.
- Effective communication, organizational and critical thinking skills.
Preferred Skills
- Education: Master’s Degree in Health Informatics, Business Administration, Health Care Administration, or related discipline required or equivalent combination of education and experience preferred.
- Certification: Certification from a national informatics certifying body preferred.
